RMA Form Instructions

Use the RMA form to request an RMA (Return Merchandise Authorization) number before returning material to CPC. When returning equipment to CPC please observe the following instructions:

    1. Request an RMA Number
      You must obtain an RMA number before returning material to CPC. This is done by completing the attached RMA form. Before submitting an RMA form, you need:
      • Model & Serial number of the product in question.
      • Name and contact information of issuer of complaint (for possible verification of fault)
      • Detailed description of the problem with the equipment
      • Date of product delivery

      CPC will notify you of your RMA # by email or phone. All RMA numbers are valid for 30 days from the issue date. Standard lead time for repairs is 4 to 6 weeks after receipt of the amplifier. Shorter lead times are available for an additional fee.

    2. Review Your Warranty

      Please note the length of the warranty of the returned item(s). If the item is out of warranty, CPC will require an evaluation fee of $2,800.00 per item. A check or purchase order will need to be received for the evaluation fee within 30 days after receiving the amplifier.

      Also note that many OEMs (Original Equipment Manufacturers) warrant their products through service contracts. Please check with the OEM for their warranty and repair policies and procedures before contacting CPC.

  • Packaging Your Equipment

    After receiving your RMA number, use the following steps to prepare your package for shipment:

    CPC packages its products in wood shipping crates or fortified cardboard boxes that are intended to be used once. New containers should be used when returning a product to the factory. If a product must be returned in used containers, CPC cannot warranty the product against shipping damage unless the inner foam and outside wood have been reinforced at all corners.

    If containers are required, check the appropriate box on the RMA form and CPC will send containers for an additional charge of $1,000.00 for a single chassis crate or $2,500.00 for rack units.

      • Each chassis unit should be placed in a container with at least 4 to 6 inches of clearance on all sides, top and bottom.
      • Fill the open container space surrounding the product with a suitable packing material such as foam rubber or foam packing material. DO NOT USE POPCORN PACKING MATERIAL!
      • To properly cushion the contents of your package, wrap each large product separately. Use proper electrostatic discharge (ESD) precautions.
      • Include inside your shipment a description of the problem and a point of contact at your company.
      • Include any accessory pieces such as interconnecting cables and AC line cords.
      • Firmly seal all boxes or crates.
  • Labeling Your Package
    When shipping a package, include your RMA number on the label. Send the package to the following address:

If the product is in warranty but CPC determines that damage was caused by mis-handling of the product (see warranty information), CPC will invalidate the warranty. The product will be treated as out of warranty.

If the item is out of warranty, CPC will require an evaluation fee of $2,800.00 per item. A check or Purchase Order will need to be received for the evaluation fee within 30 days after receiving the amplifier.

All RMA Numbers are valid for 30 days from the issue date. Failure to follow these instructions may result in your shipment being refused or your RMA not being processed.

There is a limited warranty on “repaired” items of 1 year from the Date of Repair (DOR). DOR is the date on the Test Report or Shipment documents from CPC.

  • Actual Repair

    After the evaluation has been completed, CPC shall notify you by fax or email of the expected cost of repair. If the repair cost is accepted, a purchase order or check must be received before the repaired unit will be returned. The evaluation fee of $2,800.00 will be applied to any repair costs.

    It is also acceptable for CPC to amend the original evaluation purchase order if given. CPC reserves the right to alter the cost of repair due to unforeseen damage. Any additional costs will be quoted for approval by the customer.

    If the repair cost is not accepted, CPC will either send the material back to the sender or dispose of it in accordance with WEEE (Waste of Electrical and Electronic Equipment) directives. If the sender plans on disposing of the unit themselves, it will need to be done in accordance with WEEE directives.
